I understand that the submission of this application form is not a guarantee of my acceptance for employment in Suzuki Philippines, Inc. (SPH). I understand that I still have to undergo SPH’s selection and recruitment process. SPH shall screen and evaluate my application and has the sole discretion to determine my fitness and suitability to the position I am applying for.

I hereby certify to the authenticity and completeness of all the information I supplied herein. I understand that any omission of and/or misrepresentation of the information I provided is a ground for the automatic denial of my application for employment. On the other hand, should I be offered employment, and such omission or misrepresentation be known to SPH only after my actual employment with SPH, I understand that the same shall be sufficient cause for my dismissal.

I am freely giving my informed consent to the collection and processing of my personal information and sensitive personal information for purposes of identity verification, fraud prevention, security, proper record retention and other administrative purposes in relation to my application for employment with SPH. The personal information and sensitive personal information about and/or relating to me which SPH and/or its authorized agent/s or representative/s will collect and process may include, among others, my name, signature, civil status, date and place of birth, photo, finger print, education and employment background, criminal/police records, if any, and such other data needed for administrative purposes. I understand that such information will be retained by SPH only for as long as necessary for the fulfilment of these purposes, as well as for the establishment, exercise or defense of legal claims, for other legitimate business purposes, or as provided by law. Reasonable and appropriate organizational, physical and technical measures are in place to maintain the privacy and security of such information. I further give such free and informed consent to the transmittal of said personal data to third party providers that are contracted by SPH for processing and storage in relation to my application for employment within the limits provided by existing laws.

I acknowledge that I have read and understood [Suzuki Philippines Incorporated’s Data Protection Policy for Employees] (the "Policy"), and consent to the collection, use and disclosure of my personal data by Suzuki Philippines Incorporated for the purposes set out in the Policy. I may withdraw consent for such collection, use and disclosure, and make an access or correction request in respect of my personal data, in accordance with the Policy.
